This part qualification prepares learners to work as Home-Based Care Assistants, providing supervised care and support to patients and their families in home and community settings. The goal is to promote, restore, and maintain the patient’s comfort, function, and health, including dignified end-of-life care. A qualified learner will be able to assess, plan, and provide holistic care, involve patients and family in the care process, make referrals for continuous care, and maintain accurate records of the care provided.
